I've sat in the cockpit of a MiG-23, well...also the -15, -17, -19, and -21. I missed the MiG-25 by a few months.
Just an observation, I only see one post of a Medal of Honor being awarded...
I beat this on my second attempt but I forgot to turn on the video. So I beat this a second time.

Yes, my opponent was Ace level.
I was fighting 2 vs 1 again against MiG-21s and I had a flare dispenser failure that destroyed my F-5. I didn't even know that was possible. I had to watch the Tacview to make sure the second MiG didn't fire at me.
Tacview isn't always 100% reliable. Suspect the fault is in DCS. What can fail is configurable.
In the main settings panel there is an option under the misc tab to allow random system failures. Then for specific aircraft in missions you can set which systems to fail and when. The plane type must be "player" for those options.
I've read people talking about the game getting "out of sync" and it can happen in real time in single player. Things happen that you can't see and so aren't recorded properly by TacView either. I think this does happen occasionally but thankfully it's not frequent.

The end mission screen showed a flare dispenser failure. I didn't believe it so I watched the Tacview.
In the mission settings panel you can turn on random failures for the entire mission and planes don't have to be client or player.
I have no idea how to set specific system failures and when they fail.
Here's an example of a sync issue. The first of three missions I flew F-5 vs Mig-23. I didn't get the playbacks aligned just right but you can see in Tacview he never shot me.
But I was shot down in that fight.
Had another flight a while back though in the F-5 and I was chasing a lone survivor, almost straight and level, no hard maneuvering, lost my wings. TacView playback showed enemy plane stationary in front of me and showed me ram him. Not at all what I saw on my screen.
An F-14 vs 2 Mig-23s. Tough fight. Took me several tries to kill both. Spliced the intro with the "fly again" I finally won.
How are you adding countermeasures to the F-14 loadout? Mine are greyed out for both the A and B models...
